ACADEMIC PROGRAMS
Undergraduate Programs

Entrance Examination
For Freshmen StudentsUpon application for University Entrance Examination (UEE), student applicants shall be required to submit the following:

  • Four (4) pieces of 2×2 ID picture
  • Photocopy of Form 138
  • Php400 Entrance Examination Fee

For Transferees

Upon application for University Entrance Examination (UEE), student applicants shall be required to submit the following:

  • Four (4) pieces of 2×2 ID picture
  • Transfer Credentials or Honorable Dismissal
  • Certificate of Good Moral Character
  • Certified True Copy of Grades
  • Letter of Application
  • Php400 Entrance Examination Fee
  • Original Copy of NSO Birth Certificate

For International Students

Upon application for University Entrance Examination (UEE), student applicants shall be required to submit the following:

  • Letter of lntent
  • Three (3) copies of 1998 Revised Personal History Statement (P.H.S.)
  • One (1) set of Official Transcript of Records/Scholastic Records duly authenticated by the Philippine Embassy or Consulate in the applicant’s country of origin or legal residence
  • One (1) set of Notarized Affidavit of Suppport and Proof of Adequate Financial Support duly authenticated by the Philippine Embassy or Consulate in the applicant’s country of origin
  • One (1) set of Certificate of Good Moral Character
  • One (1) set of photocopy of Passport pages where full name, photo, and birth date appear, with last arrival stamp
  • One (1) set of Police Clearance duly authenticated by the Philippine Embassy or Consulate in the applicant’s country of origin or legal residence
  • One (1) set of original of Quarantine Test (Medical Examination Result)
  • Four (4) copies of 2 x 2 ID picture
  • Php8,000 Acceptance Fee
  • Php5,000 International Fee
  • Php400 University Entrance Examination Fee
