Obtain Application Form from the Office of Admissions, your high school guidance counselor or the nearest Provincial Testing Center.
Fill out and submit the form together with 2 1×1 ID pictures to the Office of Admissions or Provincial Test Center.
Pay a fee of P185.00 for processing the application and for testing.
