Freshmen

• Form 138 or Fourth Year High School Card
• Principal’s Certificate of Good Moral Character
• Principal’s Certification of Academic Ranking for Valedictorians and Salutatorians.
• Original and Xeroxed copies of Birth and Baptismal Certificates
• Six (6) copies of 1×1 ID pictures
• Medical Certificate
• Police/NBI Clearance for those coming from outside the province.
• P150 Testing Fee

Transferees

• Transfer Credential (Honorable Dismissal)
• Xerox copy of scholastic records
• Letter of Recommendation from the Dean/Guidance Counselor
• Birth and Baptismal Certificates
• Medical Certificate
• Six (6) copies of latest pictures (1 1/2 X 1 1/2)
• Police/NBI Clearance for those coming from outside the province
• P150 Testing Fee

Cross-enrollees

• Cross registration permit from the Dean/Registrar of the student applicant’s home school
• Two (2) copies of 1 1/2 x 1 1/2 pictures
• P150 Testing Fee

Alien Students

• Official transcript duly authenticated by the Ministry of Education of the student applicant’s country
• Personal history statement (forms are supplied by the College)
• A notarized affidavit of support
• Study permit and Alien Certificate of Registration (CAR)
• Certificate of English Proficiency for applicants from non-English speaking countries
• Six(6) copies of latest picture (1 1/2 x 1 1/2)
• P150 Testing Fee
