DepEd Senior High School Voucher Program Requirements
The Department of Education’s senior high school voucher program is now approaching its third year of implementation. If you are looking into availing one for yourself to help with the tuition fee, below is the list of requirements that you need to submit.
Online Application:
- Printed Online Application Confirmation Slip
- Certification of Financial Assistance from the school, if applicable
- Recent Identical 2×2 photo (2 Copies)
- Original Philippines Statistics Authority (PSA) Certified Birth Certificate
- Photocopy of Grade 10 Report Card
- Certificate of Employment of Parent/Guardian (if Applicable).
- Latest Income Tax Return of Parent/ Legal Guardian or Certification of Tax Exemption or Municipal Certification of Unemployment
Manual Application
- Filled-out Voucher Application Form (VAF-1).
- Recent Identical 2×2 photo (2 Copies).
- Photocopy of Grade 10 Report Card.
- Certificate of Employment of Parent/Guardian (if Applicable).
- Latest Income Tax Return of Parent/ Legal Guardian or Certification of Tax Exemption or Municipal Certification of Unemployment.
Online applicants may apply via http://ovap.deped.gov.ph/ while manual applicants may send their applications to PEAC National Secretariat, 5th Floor Salamin Building, 197 Salcedo Street, Makati City.
Applications for AY 2019-2020 has not yet started so keep checking the official DepEd website for announcements.
